Obtaining Patient Consent

Donating images to the Image Archive requires that you have patient consent prior to collecting photos.

Each institution/company may have different requirements/rules for collecting patient consent. The statements below are samples of what can be included on your pateint forms, but please check with your institution/company before implementing it to ensure all legal requirements are being met.

1. I authorize the healthcare facility to examine, photograph, or preserve for teaching or research purposes, or to otherwise dispose of the tissue, limbs, organs or foreign objects resulting from the operation or procedure authorized above.
2. I authorize the above provider to take photographs or videotapes of the procedure specified above and to use the pictures for hospital records, teaching and in medical and scientific publications.
